Atorvastatin

Atorvastatin is antihyperlipidemic agent. Atorvastatin is a selective, competitive HMG-CoA reductase inhibitor. Atorvastatin is the only drug in its class specifically indicated for lowering both elevated LDL-cholesterol and triglycerides in patients with hypercholesterolemia. It prevents coronary atherosclerosis and reduces the risk of heart attack.

Atorvastatin Calcium is the derivative of Atorvastatin. It is of Synthetic origin and belongs to Pheynylcarbomoyli. It belongs to HMGCoA reductase inhibitor pharmacological group on the basis of mechanism of action and also classified in Antilipemic Agent pharmacological group.The Molecular Weight of Atorvastatin is 1208.42. Its pKa is 4.6.
Contraindications
Atorvastatin is contraindicated in conditions like Hypersensitivity,Pregnancy,Breast feeding,Actinic keratosis.
Effects
The symptomatic adverse reactions produced by Atorvastatin are more or less tolerable and if they become severe, they can be treated symptomatically, these include Flatulence, Weakness, Headache, Diarrhea, Fever, Myalgia, Constipation, Insomnia, Abdominal pain, Skin rashes, Blurred vision, Pharyngitis, Dyspepsia, Malaise, Asthenia, Arthralgia, Elevation of liver enzymes, Myopathy, Backache, Increased intracranial pressure, GI disturbance, Sinusitis.
Indications
Atorvastatin is primarily indicated in conditions like Familial hypercholesterolaemia, Homozygous familial hypercholesterolemia, Hypercholesterolaemia, Hyperlipoproteinaemia, Hypertriglyceridaemia, NSAID-associated gastric and duodenal ulcers and duodenal errosions.
Interactions
Atorvastatin is known to interact with other drugs, the details of drug interactions is as follows:DrugDetailsSeverityOnsetManagementAliskirenAmprenavirAmprenavir possibly increases plasma concentration of Atorvastatin. ADVICE: Avoid concomitant use. MajorCimetidine (HCl)Cimetidine may decrease the metabolism of Atorvastatin. ModeratePatients who require concomitant thaerapy should be monitored for altered antihypertensive response if Cimetidine is introduced or discontinued, or when its dosage is modified.Colestipol (HCl)Cyclosporin ACyclosporin may increase the serum concentration of Atorvastatin. MajorMonitor altered effects (eg, increased serum CPK, myopathy, rhabdomyolysis) of Atorvastatin if Cyclosporin is introduced or discontinued, or when its dosage is modified.DigoxinAtorvastatin possibly increases plasma concentration of Digoxin.ErythromycinErythromycin may decrease the metabolism, via CYP isoenzymes, of Atorvastatin. Possible increased risk of myopathy when atorvastatin given with erythromycin.ModerateWhen coadministration of a Macrolide antibiotic and an HMG-CoA reductase inhibitor is necessary, consider using a combination which appears to hold the lowest risk for the development of rhabdomyolysis. Azithromycin, Dirithromycin, and Spiramycin can probably be used safely (ie, no increased risk of side effects) with any HMG-CoA reductase inhibitor. Neither Fluvastatin, Pravastatin, nor Rosuvastatin would likely be affected by Macrolides.Mibefradil (Di HCl)Nicotinic AcidIncreased risk of myopathy when statin (e.g Atorvastatin) given with nicotinic acid (applies to lipid regulating doses of nicotinic acid.RepaglinideAtorvastatin may increase repaglinide concentrations by decreasing metabolism. Warfarin (Na)Atorvastatin may enhance the anticoagulant effect of Warfarin. Moderate (Sequence important)Patients who require concomitant thaerapy should be monitored for altered response whenever Warfarin is introduced or discontinued, or when its dosage is modified. Dosage adjustments of Warfarin may be needed. These interactions are sometimes beneficial and sometimes may pose threats to life. Always consult your physician for the change of dose regimen or an alternative drug of choice that may strictly be required.
Interfrence
Risks
Drug should not be given to Paediatrics, Pregnant Mothers, and patients suffering from Liver Malfunction.If prescribing authority justifies the benefits of the drug against the possible damages he/she should reevaluate them and consult the reference material and previous studies.
Storage
Tab Store in a well closed container, Between 20°C-25°C. Protect from Moisture and Heat.
Warnings
Atrovastatin should be used with caution in patients with liver disease, severe infection, heart disease, thyroid problems, uncontrolled seizures, recent major surgery, low blood pressure, any allergy or any other medical problem. Atrovastatin may cause dizziness or lightheadedness, so patient should observe caution while driving or engaging in activities requiring alertness. Atorvastatin should not be used during pregnancy or lactation.
